Mandates licensed health care providers to give parents, legal guardians, or legal representatives a copy of the current CDC vaccine information statement before administering any vaccine required for school entry

Requires parents, legal guardians, or legal representatives to sign a statement confirming they received the vaccine information, understand the benefits and risks, and consent to the vaccination

Health care providers must record the batch and lot number of each vaccine on the signed statement and maintain a copy in the child's permanent medical record

Children must commence the required vaccination schedule before beginning kindergarten or initial entry into public or private school, whichever occurs earlier, with an effective date of July 1, 2010
