Requires operator or proprietor of a tanning facility to witness the signing of a written parental/guardian consent statement before a minor 14 years or older can use a tanning device

Written statement must include parent or legal guardian acknowledgment that they have read warnings, consent to use, agreement that minor will use protective eyewear, and authorization for a specified number of tanning sessions within a 12-month period not to exceed department rules or manufacturer schedules

Prohibits minors younger than 14 years of age from using tanning devices at facilities, with exception for use prescribed by a health care provider (which still requires parental consent statement compliance)

Requires tanning facility operators to refuse service to minors whose parent or legal guardian has not signed the required statement in the operator's presence

Effective date is July 1, 2010
