Eliminates the separate "limited nursing services" license category, allowing facilities with a standard license to provide limited nursing services directly through licensed personnel.

Increases biennial license fees for standard licenses from $300 to $356 base fee plus $67.50 per resident (maximum $18,500, up from $10,000), and increases extended congregate care license fees from $400 to $501 base fee.

Removes requirement for Agency for Health Care Administration to conduct quarterly monitoring visits of extended congregate care facilities and eliminates the Department of Elderly Affairs annual reporting requirement on extended congregate care services.

Requires facilities with Class I or Class II violations cited within the previous 24 months to undergo periodic unannounced monitoring, with registered nurse participation required in at least two onsite visits annually for violations related to nursing care.

Mandates licensed facilities to report semiannually to the Agency for Health Care Administration on resident census, service types, funding sources, and staffing, with specific rules to be adopted by the Department of Elderly Affairs.
