Establishes an office within the Division of Children's Medical Services Network to oversee and implement health care transition programs for adolescents and young adults aged 12-26 with chronic physical, developmental, behavioral, or emotional conditions requiring specialized health care.

Requires the office to develop planning processes, resource guides, training coordination, web-based materials, and quality improvement components for health care providers and communities implementing transition services.

Directs the Department of Health to partner with the Agency for Health Care Administration to identify waiver and state plan options for provider compensation strategies including co-management, care plan development, medical home provision, and telemedicine to improve access to adult and specialty care.

Authorizes the Department of Health to contract with local health and transition services programs that include partnerships between adult and pediatric providers, community-based health services, support organizations, and medical homes; at least one program must operate in each of the eight Children's Medical Services Network regions.

Requires independent evaluation of local programs by an organization or university with health care program evaluation expertise, using formative, cumulative measures of program process, outcomes, and client results.
