Authorizes the Agency for Health Care Administration to seek a federal Medicaid waiver under Section 1115 of the Social Security Act to implement a 5-year pilot project improving health care access in rural counties (defined as counties with fewer than 85,000 residents).

Requires pilot program contracts of 3-5 years in participating counties to include medical home models, risk-adjusted rates, electronic health records development, cost savings demonstrations, and toll-free complaint lines.

Permits rural county pharmacies to purchase drugs at the same prices as federally qualified health centers serving Medicaid recipients and allows the board of county commissioners to develop county-specific or regional requests for proposals in consultation with the agency.

Establishes local health advisory councils in participating counties to review encounter data, process consumer complaints, and conduct monthly public meetings while including an agency representative as a nonvoting member.

Requires the agency to submit a progress report to the President of the Senate and Speaker of the House by February 1, 2011, with an effective date of July 1, 2010.
