Requires the Agency for Health Care Administration to establish a medical home pilot project with three tiers of primary care providers (Tier One, Two, and Three) offering increasingly comprehensive care coordination services for Medicaid beneficiaries.

Creates a task force by August 1, 2009, and establishes a seven-member statewide advisory panel appointed by legislative leaders and the Governor to oversee development and implementation of the program.

Establishes enhanced case management fee structures: Tier One at 110 percent of standard Medipass fees, Tier Two at 130 percent of Tier One fees, and Tier Three at 200 percent of Tier One fees, with adjustments based on patient age, gender, and eligibility.

Directs the agency to identify priority areas based on emergency department use and hospitalization rates, designate at least one medical home per priority area by October 1, 2010, and contract with the University of South Florida to evaluate the program's impact on utilization and spending.

Allows medical home networks achieving savings equal to or greater than prepaid health plan costs to receive funding distributed as fee multipliers to participating providers, with intent to raise Medicaid physician fees to 100 percent of Medicare rates.
