Motor vehicle registration forms and driver license application forms must include an option for voluntary $1 contributions to aid the homeless, with funds deposited into the Grants and Donations Trust Fund of the Department of Children and Family Services.

Creates a homelessness prevention grant program providing emergency financial assistance to families facing loss of shelter, with maximum grants of $300,000 per lead agency and program administration costs capped at 3 percent.

Requires lead agencies to track and monitor assisted families for at least 12 months, with a goal that at least 85 percent of assisted families remain housed and avoid homelessness within one year.

Limits administrative costs for Challenge Grants to a maximum of 8 percent of funding and requires housing assistance grant funding to be appropriated as fixed capital outlay items.

Repeals the emergency assistance program for families with children that lost or faced loss of shelter under section 414.16 and deletes a related cross-reference in section 420.625.
