Expands the nursing home diversion program statewide to include all Medicaid recipients age 60 or older who meet nursing home admission criteria and are dually eligible for Medicare and Medicaid, replacing the previous pilot program approach.

Implements the program in phased rollout: Areas 5, 6, and 7 initially, Areas 8, 9, 10, and 11 in 2013, and Areas 1, 2, 3, and 4 in 2014, with Medicaid recipients given choice of contractors in each area.

Requires nursing home diversion contractors to inform enrollees of feasible alternatives and offer choice between institutional care or home and community-based services when level-of-care determination is made.

Consolidates all Medicaid funding for eligible individuals into the integrated program, including home and community-based waiver services, nursing home services, and Medicare cost-sharing for dually eligible persons.

Excludes individuals enrolled in developmental disabilities, AIDS care, traumatic brain injury, consumer-directed care waivers, and Program of All-Inclusive Care for the Elderly (PACE) from the integrated program; requires Office of Program Policy Analysis to evaluate program by December 31, 2014.
