Prohibits health insurers from requiring contracted health care practitioners to accept terms of other contracts with the insurer, other insurers, or health maintenance organizations under common management and control, except for practitioners in group practices who must accept group-negotiated contract terms.

Prohibits health insurers from requiring dentists to provide services at insurer-set fees unless the services are covered services under the applicable contract.

Defines "covered services" as services reimbursable under the contract subject to contractual limitations like deductibles, coinsurance, and copayments, but excludes services provided after annual or periodic payment maximums are met and services not listed as covered benefits.

Applies the same dentist fee-setting restrictions to prepaid limited health service organizations and health maintenance organizations under their respective contract provisions.

Takes effect July 1, 2011.
