Amends sections 626.9541 and 641.3903 of Florida Statutes to authorize insurers and health maintenance organizations to offer voluntary wellness or health improvement programs with rewards and incentives for participation.

Allows rewards or incentives including merchandise, gift cards, debit cards, premium discounts, health savings account contributions, or modifications to copayments, deductibles, or coinsurance amounts.

Permits insurers and HMOs to require members who do not participate in wellness programs to provide physician verification that their medical condition makes participation unreasonably difficult or inadvisable in order to receive rewards or incentives.

Requires that any offered rewards or incentives be disclosed in the policy or certificate provided to members.

Takes effect July 1, 2011.
