Requires Department of Education to include in parent guide guidelines notification that parental permission is required for students to participate in reproductive health or sexually transmitted disease education, including HIV/AIDS instruction.

Authorizes school districts to include a parental consent form in the parent guide distributed to parents and students.

Changes reproductive health and disease education from an opt-out system (parent requests exemption) to an opt-in system (requires parental permission for student participation).

Revises comprehensive health education requirements to emphasize nutrition, physical fitness, substance abuse prevention, and the connection between physical well-being and mental/emotional health.

Students who do not receive parental permission for reproductive health or disease education cannot be penalized for lack of permission.
