Requires tanning facility operators to witness the signing of parental/legal guardian consent statements before allowing minors to use tanning devices.

Changes age eligibility from 14-18 year-olds to only 14 years and older, prohibiting minors younger than 14 from using tanning devices except when medically prescribed.

Expands parental consent statement requirements to include: acknowledgment of warnings, consent to tanning use, agreement on protective eyewear use, and authorization of a specific number of tanning sessions within a 12-month period not exceeding departmental rules or manufacturer guidelines.

Mandates facility operators refuse service to minors unless the parent or legal guardian has signed the required statement in the operator's presence.

Allows minors younger than 14 to use tanning devices only if prescribed by a health care provider, with parental compliance with all consent requirements still required.

Takes effect July 1, 2011.
