Creates section 790.338 establishing that licensed health care practitioners and facilities may not record firearm ownership information in medical records unless relevant to the patient's medical care or safety or the safety of others.

Prohibits health care practitioners and facilities from inquiring about firearm ownership or possession unless the information is relevant to patient care or safety, with exceptions for emergency medical technicians and paramedics in medical emergencies.

Grants patients the right to decline to provide information regarding firearm ownership or possession, and prohibits health care practitioners or facilities from discriminating against or harassing patients based solely on firearm ownership.

Prohibits insurance companies from denying coverage, increasing premiums, or otherwise discriminating based on lawful firearm or ammunition ownership or use, while permitting consideration of fair market value of firearms in personal property coverage premiums.

Establishes violations of these provisions as grounds for disciplinary action against health care practitioners and amends the Florida Patient's Bill of Rights to include protections regarding firearm ownership privacy.
