Establishes the Florida Health And Transition Services (FloridaHATS) program within the Division of Children's Medical Services Network to develop and implement health care transition services for individuals aged 12-26 with chronic physical, developmental, behavioral, or emotional conditions requiring specialized care.

Requires FloridaHATS to oversee program implementation, develop planning guides for communities, disseminate resource guides on health care financing options, maintain a website, and provide technical assistance to providers and organizations.

Directs the Department of Health to partner with the Agency for Health Care Administration to identify waiver options and provider compensation strategies, including co-management, care plan development, medical homes, and telehealth technology.

Requires the Department of Health to work with the Office of Insurance Regulation to develop policies based on a patient-centered medical home model and to collaborate with pediatric and adult health care providers to establish at least one local transition services program in each of the eight Children's Medical Services Network regions.

Specifies that local programs must include partnerships between adult and pediatric providers, community-based health services, support organizations, and medical homes, and must offer assessments, transition planning, and disease self-management programs.
