Motor vehicle registration and driver license application forms must include an option for a voluntary $1 contribution to aid the homeless, with funds deposited into the Grants and Donations Trust Fund of the Department of Children and Family Services for use by the State Office on Homelessness.

Establishes a homelessness prevention grant program providing emergency financial assistance to families facing loss of home, with maximum grants of $300,000 per lead agency and administrative costs capped at 3 percent of the grant award.

Requires lead agencies to track and monitor assisted families for at least 12 months and achieve a goal of keeping at least 85 percent of assisted families housed and avoiding homelessness during the following year.

Limits lead agencies to spending a maximum of 8 percent of funding on administrative costs and repeals the existing emergency assistance program for families with children who lost or face loss of shelter.

Revises the Housing First approach methodology by removing requirements for background checks and addiction rehabilitation as prerequisites for housing assistance.
