Creates requirements for health insurers and HMOs to provide coverage for FDA-approved prescription contraceptive drugs and devices, as well as outpatient contraceptive services including insertion, removal, examinations, consultations, and related medical services.

Prohibits health insurance policies and HMO contracts from imposing unusual copayments, coinsurance, deductibles, or waiting requirements for contraceptive drugs, devices, or services unless such cost-sharing is applied to all other healthcare practitioner services.

Allows religious employers to request and requires insurers and HMOs to grant an exclusion from contraceptive coverage if it conflicts with the employer's bona fide religious beliefs and practices, with notice required to employees.

Applies to accident and health insurance policies, group/blanket/franchise policies, and health maintenance contracts issued, amended, delivered, or renewed in Florida.

Takes effect July 1, 2011, and applies only to contracts and policies issued or renewed on or after that date.
