Expands the Florida Health Choices Program to allow employers with 1-50 employees to make employees eligible for health products, and adds statutory rural hospitals as eligible employers rather than participants.

Permits prepaid limited health service organizations and discount medical plan organizations to sell products through the program, and requires risk-bearing products offered by insurers to be approved by the Office of Insurance Regulation.

Requires Florida Health Choices, Inc. to operate a toll-free hotline for assistance, establish initial and special enrollment periods not exceeding 60 days, and report risk pooling data annually.

Authorizes the Florida Health Choices, Inc. to receive personal identifying information of Florida Kidcare program applicants and enrollees from the Agency for Health Care Administration for program administration purposes.

Requires the Agency for Health Care Administration to establish a demonstration project in Miami-Dade County including both a long-term-care facility and psychiatric facility to improve health care access for medically underserved populations.
