SB 2034 - Independence at Home Act

• Establishes the "Independence at Home Chronic Care Coordination Pilot Project" to provide primary care services in beneficiaries' homes for high-cost Medicaid patients with multiple chronic conditions, with phased implementation beginning July 1, 2012.

• Requires participating organizations to achieve minimum 5 percent cost savings annually through reduced hospitalizations, emergency room visits, and nursing home admissions, with savings calculations verified by independent research organizations using econometric models.

• Mandates at least two unaffiliated Independence at Home organizations per county during initial 3-year phase, with expansion possible after independent evaluation if organizations meet quality, performance, and participation standards.

• Specifies eligible beneficiaries must have two or more qualifying chronic conditions (including congestive heart failure, diabetes, COPD, dementia, stroke, and others), functional impairment requiring assistance with three or more daily activities, and prior hospital or emergency department utilization within 12 months.

• Establishes 80 percent revenue sharing for savings achieved during initial phase and up to 80 percent for savings up to 25 percent above requirements during expanded phase, with remainder retained by Medicaid program.
