Establishes the Florida Health And Transition Services (FloridaHATS) program within the Division of Children's Medical Services Network to oversee health care transition services for individuals ages 12-26 with chronic physical, developmental, behavioral, or emotional conditions requiring specialized care.

FloridaHATS must develop planning guides, resource materials, training coordination, website content, and quality improvement measures for communities and providers implementing transition services that integrate health care with education, vocational rehabilitation, and independent living programs.

Department of Health must partner with the Agency for Health Care Administration to identify Medicaid waiver and state plan options for provider compensation strategies including co-management, care plan development, medical home services, and telehealth technology.

Department of Health must collaborate with the Office of Insurance Regulation to recommend policies based on patient-centered medical home models for adolescents and young adults with special health care needs.

At least one local health and transition services program must be established in each of the eight regions of the Children's Medical Services Network, featuring partnerships between pediatric and adult providers, community-based health services, support organizations, and a medical home model; programs must offer health assessments, transition planning, and disease self-management services and undergo independent evaluation.
