Removes requirement that notice of exemption election be in written form and notarized under oath for officers in the construction industry seeking workers' compensation exemptions.

Expands eligibility for workers' compensation exemption elections to include members of limited liability companies, in addition to sole proprietors and partners not in the construction industry.

Changes exemption notice submission requirements to allow electronic submission and modifies required information to include date of birth and Florida driver's license/identification card number instead of social security number.

Extends applicability of the 2-year certificate validity period to all certificates of exemption issued on or after January 1, 2013, not just those in the construction industry.

Requires the Department of Financial Services to notify certificateholders of expiration at least 60 days in advance via mail or email, removing the requirement to send renewal applications with the expiration notice.
