Extends the lookback period for calculating workers' compensation penalties from 1 year to 3 years for employers who fail to secure required coverage.

Establishes tiered penalty multipliers based on violation duration: 1.5 times premium for violations under 30 days, 1.75 times for 30 days to 1 year, and 2 times for violations lasting 1 year or more.

Minimum penalty of $1,000 applies regardless of calculated penalty amount.

Classifies any subsequent violation occurring within 5 years of a prior violation as a knowing act under Florida Statute 440.105, subjecting employers to enhanced penalties.

Takes effect July 1, 2012.
