Exempts personal identifying information from dental workforce surveys submitted by licensed dentists or dental hygienists to the Department of Health from public records disclosure requirements under Florida law and the state constitution.

Allows disclosure of exempted information only with written consent of the individual, by court order for good cause, or to approved research entities under a data-use agreement with restrictions on identifying information and use.

Authorizes the Department of Health to deny research requests if the protocol includes intrusive follow-back contacts, lacks plans for record destruction, is administratively burdensome, or lacks scientific merit.

Subjects the exemption to the Open Government Sunset Review Act and requires legislative reenactment by October 2, 2017, to prevent automatic repeal.

Establishes that maintaining confidentiality of survey respondent information is necessary to ensure candid responses and support the state's interest in maintaining adequate dental workforce availability.
