Requires individual accident or health insurance policies issued or renewed on or after January 1, 2013 to provide coverage for orthoses, prostheses, orthotics, and prosthetics equal to federal Medicare benefits under 42 U.S.C. sections 1395k, 1395l, and 1395m.

Coverage is subject to deductible and coinsurance provisions applicable to outpatient visits, but insurers must offer an optional rider without deductible or coinsurance for an additional premium.

Allows insurers to require prior authorization for orthoses and prostheses and to limit coverage to the most appropriate model as determined by the treating physician; repair and replacement are covered except when caused by misuse or loss.

Extends the same coverage requirements to group, blanket, and franchise health insurance policies and health maintenance organization contracts issued or renewed on or after January 1, 2013.

Does not apply to limited benefit policies including hospital confinement indemnity, disability income, accident only, long-term care, Medicare supplement, and specified disease indemnity coverage; effective July 1, 2012.
