Requires tanning facility operators to witness a parent or legal guardian's signature on a written consent statement before allowing minors aged 14-17 to use tanning devices

Written statement must confirm that the parent/guardian has read warnings, consents to use, agrees the minor will wear protective eyewear, and authorizes a specific number of sessions (not to exceed department rules or manufacturer guidelines) during a 12-month period

Prohibits minors younger than 14 years of age from using tanning devices at facilities

Exempts tanning device use prescribed by a health care provider from the age restriction, though parent/guardian must still comply with written consent requirements
