Establishes the Medicaid Pilot Private Care Program (MPPCP) in Alachua, Levy, Gilchrist, Dixie, and Union Counties with a 5-year federal waiver from standard Medicaid mandates to implement a private-public health insurance policy purchase model.

Requires enrollment of 20 percent of eligible Medicaid enrollees per year, with all enrollees in the pilot area receiving private care plan coverage or a hybrid state-private plan within 5 years.

Mandates enrollees purchase cost-effective hospitalization-only policies from state-approved private insurance providers and comply with drug testing, family counseling, financial planning courses, and full-time employment or educational pursuit requirements.

Creates a program oversight committee composed of representatives from hospitals, physicians, insurers, state actuarials, and the pilot counties to establish metrics comparing Medicaid costs and outcomes with private insurance and to develop reinsurance programs.

Requires Medicaid enrollees not participating in MPPCP to purchase commercial hospitalization-only policies from specified providers including Shands Teaching Hospital, Hospital Corporation of America, and participating Medicaid physicians; takes effect July 1, 2012.
