Updates cross-references in section 766.314(5) from "paragraphs" to "paragraph" to reflect the single remaining paragraph (4)(b) for ongoing annual assessments and payment obligations

Updates cross-reference in section 766.316 from section 766.314(4)(c) to 766.314(4)(b) regarding notice requirements to obstetrical patients about plan participation
