Establishes the Florida Health And Transition Services (FloridaHATS) program within the Division of Children's Medical Services Network to oversee transition services for adolescents and young adults with special health care needs.

Defines target population as individuals aged 12-26 years with chronic physical, developmental, behavioral, or emotional conditions requiring health care services beyond those typically needed.

FloridaHATS shall develop planning guides for providers, create resource guides on health care financing options, coordinate with educational institutions on training programs, and maintain a website with transition-related materials.

Requires FloridaHATS to collect and disseminate evidence-based practices, provide technical assistance to communities and providers, establish quality improvement measures, and create a network of medical experts for guidance on best practices.

Takes effect July 1, 2012.
