Prohibits minors under 14 years of age from using tanning devices at tanning facilities, with an exception for medically prescribed use.

Requires the operator or proprietor of a tanning facility to witness the parent or legal guardian's signature on a written consent statement before minors 14 and older may use tanning devices.

Requires the parental consent statement to include acknowledgment of warnings, consent to tanning use, agreement to use protective eyewear, and authorization of a specific number of tanning sessions not to exceed limits set by department rules and manufacturer schedules.

Mandates tanning facility operators refuse service to minors if the parental consent statement has not been signed in the operator's presence.

Takes effect July 1, 2012.
