Requires health insurers and health maintenance organizations to cover FDA-approved prescription contraceptive drugs and devices and related outpatient contraceptive services under the same terms as other benefits.

Prohibits insurers and HMOs from imposing unusual copayments, coinsurance, deductibles, or waiting periods for contraceptive drugs, devices, or services unless similar cost-sharing applies to other healthcare services.

Allows religious employers to request and requires insurers and HMOs to grant an exclusion from contraceptive coverage if it conflicts with the organization's bona fide religious beliefs and practices.

Requires religious employers that obtain an exclusion to provide employees with reasonable and timely notice of the exclusion.

Effective July 1, 2012, applying only to contracts and policies issued or renewed on or after that date.
