Requires individual health insurance policies issued or renewed on or after October 1, 2012, to cover up to $200 for noninvasive cardiovascular screening tests for detecting atherosclerotic cardiovascular disease (ASCVD).

Requires group, blanket, and franchise health insurance policies issued or renewed on or after October 1, 2012, to provide the same $200 coverage for noninvasive cardiovascular screening tests.

Requires health maintenance contracts issued or renewed on or after October 1, 2012, to provide the same $200 coverage for noninvasive cardiovascular screening tests.

Covers eligible males ages 45-75 and females ages 55-75 who have two or more ASCVD risk factors including high blood pressure, high cholesterol, low HDL cholesterol, smoking history, or family history of early ASCVD.

Covers CT scanning measuring coronary artery calcification and ultrasonography measuring carotid intima-media thickness, performed every 5 years (or every 2 years if abnormal results) by licensed laboratories with performance-based certification.
