Expands workers' compensation exemption eligibility to include members of limited liability companies, allowing them to elect exemption status similar to sole proprietors and partners.

Modifies notice requirements for construction industry officers electing exemption by allowing electronic submission instead of requiring written, notarized notices and changing identification requirements to include date of birth and driver's license/identification card number instead of social security number.

Removes requirement for officers to submit copies of employment status documentation with the IRS and occupational licenses, instead requiring only the percentage of ownership evidencing the required ownership stake.

Establishes that construction industry exemption certificates are valid for 2 years and requires the department to send expiration notices at least 60 days before expiration; effective January 1, 2013, expands this 2-year validity requirement to all exemption certificates, not just construction industry ones.

Requires the department to notify workers' compensation carriers when exemption certificates are revoked and notifies certificateholders via address on file or email address on file with the department.
