Saves exemptions from public records requirements for personal identifying information in physician workforce surveys submitted to the Department of Health, removing the scheduled October 2, 2012 repeal date under the Open Government Sunset Review Act.

Applies to personal identifying information from physicians licensed under chapters 458 and 459 who submit workforce surveys as a condition of license renewal.

Maintains three permitted disclosure methods: with express written consent of the individual, by court order upon showing of good cause, and to research entities with approved protocols and data-use agreements.

Makes conforming changes to sections 458.3193 and 459.0083, Florida Statutes, by restructuring subsection numbering and removing sunset review language.

Takes effect October 1, 2012.
