Creates statute 1008.347 requiring prekindergarten through grade 5 teachers to monitor and evaluate parental involvement on attendance, communication responsiveness, and submission of complete emergency/medical information.

Defines causes of student underachievement including inadequate rest, improper clothing, incomplete homework, lack of school supplies, tardiness, and infrequent parent-teacher communication.

Mandates teachers send parental involvement evaluations rated as "satisfactory," "needs improvement," or "unsatisfactory" with quarterly student assessments, with "needs improvement" triggered by one incident and "unsatisfactory" by two or more incidents in a quarter.

Requires district school boards to establish a dispute process allowing parents to challenge evaluations through discussions with the principal and teacher to receive feedback on improvement steps.

Directs district school boards to annually report parental involvement evaluation data to the Department of Education, which must submit a statewide report to the Governor and legislature by August 1 each year, with implementation beginning in the 2013-2014 school year.
