Adds voluntary $1 contributions to aid the homeless on motor vehicle registration, driver's license applications, and license renewal forms, with funds deposited into the Grants and Donations Trust Fund of the Department of Children and Family Services.

Creates a homelessness prevention grant program providing emergency financial assistance to families facing loss of housing, with maximum grants of $300,000 per lead agency and administrative costs capped at 3 percent.

Requires lead agencies to track and monitor assisted families for at least 12 months after assistance ends, with a goal that at least 85 percent of families remain housed and avoid homelessness during the ensuing year.

Limits lead agencies to spending a maximum of 8 percent of funding on administrative costs and removes background check and addiction rehabilitation requirements from the Housing First approach to homelessness.

Repeals the existing emergency assistance program for families that have lost shelter or face loss of shelter due to an emergency, effective July 1, 2012.
