Requires physicians to sign an affidavit before performing abortion stating they are not doing so because of the child's sex or race and have no knowledge the pregnancy is being terminated for those reasons.

Prohibits knowingly performing abortion based on the fetus's sex or race, using force to coerce sex or race-selection abortion, or soliciting payment to finance such procedures.

Creates civil liability allowing married fathers or maternal grandparents (if mother is under 18) to sue for appropriate relief including monetary damages for psychological, physical, or financial injuries.

Imposes civil fines up to $10,000 on physicians, nurses, counselors, or mental health professionals who knowingly fail to report violations to law enforcement.

Exempts women from criminal prosecution or civil liability for violations; establishes criminal felony penalties of third degree (or second degree if resulting in woman's death) for persons who willfully perform such abortions.
