Amends sections 458.3193 and 459.0083 of Florida Statutes to permanently save exemptions from public records disclosure for personal identifying information in physician workforce surveys submitted to the Department of Health.

Removes the scheduled repeal dates (October 2, 2012) for confidentiality exemptions covering workforce survey data from physicians and osteopathic physicians.

Maintains three existing disclosure exceptions: with written consent of the individual, by court order upon good cause, and to approved research entities under restricted data-use agreements.

Restructures statutory language by converting subsection designations to section-level numbering and reorganizing paragraph references without changing substantive exemption provisions.

Takes effect July 1, 2012.
