Creates mandatory statutory monetary assessments that must be imposed and included in court judgments regardless of whether announced in open court.

Establishes that when an assessment prescribes a minimum and maximum amount, the minimum assessment is presumed and shall be imposed unless the court specifies a greater amount.

Requires clerks of court to develop a uniform form by October 1, 2012, to identify and impose all statutory assessments, subject to Supreme Court approval.

Revises clerk reporting requirements to the Legislature on assessed and collected court-related fines, penalties, fees, costs, and charges, including separate reporting of waived or suspended assessments and collection rates.

Transfers responsibility for developing the court fee manual and reporting guidelines from the Department of Financial Services to the clerks of court, effective July 1, 2012.
