Requires health care practitioners (physicians, advanced practice nurses, and physician assistants) to publish and post a schedule of charges for medical services in their offices with specific formatting requirements (minimum 15 square feet, text describing services filling at least 12 square feet).

Extends charge schedule publication requirements to urgent care centers, ambulatory surgical centers, and diagnostic-imaging centers, with penalties of up to $1,000 per day for non-compliance.

Prohibits insurers from billing patients for emergency medical services and care regardless of whether a contract exists between the insurer and provider; prohibits billing for nonemergency services in contracted hospitals provided by non-contracted providers when the patient had no choice of alternative providers.

Requires hospitals to disclose to patients which providers will treat them and which providers are not contracted with the patient's insurer, including the providers' contact information; requires non-contracted providers to disclose in writing whether they will bill patients directly.

Amends multiple statutory definitions and cross-references to reflect the creation of "diagnostic-imaging center" as a new facility type and renumbering of definitions in chapter 395.
