Maintains state surcharge of $2.00 per day on motor vehicle rentals (first 30 days only) and allocates proceeds: 80% to State Transportation Trust Fund, 15.75% to Tourism Promotional Trust Fund, and 4.25% to Florida International Trade and Promotion Trust Fund.

Authorizes counties to impose an additional local surcharge of $3.00 per day on rental vehicles, subject to approval by countywide referendum and limited to the first 30 days of rental.

Requires counties to submit certified ordinances to the department within 10 days of referendum approval (by November 16) or face a one-year delay; local surcharge effective date is January 1 following approval and must terminate on December 31.

Directs local surcharge proceeds to the Local Option Fuel Tax Trust Fund for distribution to counties under fuel tax provisions, with special provision for counties with interlocal agreements with regional transportation authorities.

Exempts vehicles rented at no charge for maintenance or repair, and requires dealers to report surcharge collections by county where the rental agreement was entered into.
