Prohibits health insurers, prepaid limited health service organizations, and health maintenance organizations from requiring dentists to provide services at insurer-set fees unless those services are covered benefits under the contract.

Defines covered services as those explicitly listed as benefits that the insured or subscriber is entitled to receive, and prohibits providing merely de minimis reimbursement or coverage to circumvent these requirements.

Requires that fees for covered services be set in good faith and must not be nominal.

Prohibits health insurers and health maintenance organizations from requiring dentists to participate in discount medical plans as a condition of contract.

Effective July 1, 2013, and applies to contracts entered into or renewed on or after that date.
