Exempts personal identifying information from dental workforce surveys submitted by licensed dentists or dental hygienists to the Department of Health from public records disclosure requirements under Florida law and the state constitution.

Allows disclosure of survey information with express written consent of the individual, by court order upon showing of good cause, or to approved research entities that maintain data-use agreements with the department and follow specified protocols.

Permits the Department of Health to deny research requests if protocols include intrusive follow-back contacts, lack plans for destroying confidential records, are administratively burdensome, or lack scientific merit.

Requires research agreements to restrict release of identifying information, limit use to approved protocols, and prohibit other uses; copies of released records remain department property.

Subjects the exemption to the Open Government Sunset Review Act and automatically repeals it on October 2, 2018, unless the Legislature reenacts it.
