Creates a homelessness prevention grant program that provides emergency financial assistance to families facing loss of shelter, with grants up to $300,000 per lead agency and a 3 percent cap on program administration costs.

Establishes a competitive grant application process with preference for applicants who leverage additional public and private funds and demonstrate effectiveness in keeping families housed.

Requires lead agencies to track and monitor assisted families for at least 12 months after assistance, with a goal of keeping at least 85 percent of assisted families in their homes.

Repeals the emergency assistance program for families under section 414.16 and transfers remaining balances to the new homelessness prevention grant program.
