Requires school districts to provide one class period per day of physical education for one semester each year for students in grades 6 through 8, beginning with the 2009-2010 school year.

Allows waiver of physical education requirements for one semester each year if a student is enrolled in a remedial course.

Allows waiver if a parent writes to the school principal or designee verifying that the student participates in physical activities outside school equal to or exceeding the mandated requirement.

Adds new waiver criterion allowing students to submit a physician's note (from doctors licensed under chapters 458 or 459) specifying health reasons for not participating in physical education.

Requires school districts to notify parents of available waiver options before scheduling students for physical education, effective July 1, 2013.
