Requires health care providers to ask parents or legal guardians of children with identified hearing loss to provide contact information (mailing address or email) to receive information from registered service providers.

Directs the Department of Health, in cooperation with the Department of Education, to develop standards and register service providers offering diagnostic services, speech pathology, auditory amplification, auditory-oral education, and listening and spoken language specialist services to deaf or hard of hearing children.

Allows parents or guardians to request services from registered providers, with service levels determined by the child's individualized education program or individual and family service plan.

Establishes eligibility for services until the end of the school year in which a child reaches age 7 or after completion of grade 2, whichever occurs first, with funding based on the base student allocation multiplied by the support level V cost factor under the Florida Education Finance Program.

Authorizes the Department of Health to adopt rules implementing these requirements and effective July 1, 2013.
