Establishes licensing fees not to exceed $100-$200 for various purposes including application ($100), examination ($200), initial licensure ($200), and biennial renewal ($200).

Prohibits unlicensed personal training for compensation, misuse of licenses, employment of unlicensed trainers, and use of the "personal trainer" title without licensure; violations constitute first-degree misdemeanors.
