Maintains the existing state rental car surcharge of $2.00 per day on motor vehicle leases and rentals designed to carry fewer than nine passengers, applying only to the first 30 days of any lease or rental.

Allocates state surcharge proceeds as follows: 80% to State Transportation Trust Fund, 15.75% to Tourism Promotional Trust Fund, and 4.25% to Florida International Trade and Promotion Trust Fund.

Authorizes counties to impose an additional local surcharge of up to $3.00 per day on rental vehicles, subject to approval by countywide referendum, with effective dates of January 1 following referendum approval.

Requires counties to notify the department within 10 days of referendum approval and by November 16 before surcharge implementation; failure to notify results in a one-year delay of the surcharge effective date.

Directs local surcharge proceeds to the Local Option Fuel Tax Trust Fund for regional transportation purposes, with monthly distribution to counties that have interlocal funding agreements with regional transportation authorities.
