Amends the Florida Patient's Bill of Rights and Responsibilities to add patient responsibility for reviewing a document indicating they may be charged for out-of-network physician services.

Requires health care providers and facilities to present patients with written notice of potential out-of-network charges before services are rendered.

Allows patients to sign the out-of-network disclosure document, or permits medical facility employees to sign it to verify the patient was presented with the information if the patient declines to sign.

Modifies section 395.301 of Florida Statutes to establish the requirement for presenting the out-of-network charge notification form prior to service delivery.

Takes effect January 1, 2015.
