Requires the Agency for Health Care Administration to revoke or deny a nursing home license if the licensee or controlling interest operates a facility with two moratoria for substandard quality of care within any 30-month period (changed from 60-month period).

Expands license revocation requirements to include facilities conditionally licensed for 90 or more continuous days, or facilities cited for two class I deficiencies in related or separate circumstances within 30 months.

Imposes a $1 million civil penalty if the agency determines a resident died from abuse or neglect, replacing standard class I deficiency penalties of $10,000-$15,000.

Allows licensees or controlling interests to present mitigating factors, giving the agency discretion to decline revocation if appropriate under the circumstances.

Takes effect July 1, 2014.
