Amends Florida's Patient's Bill of Rights to add that patients are responsible for reviewing a document informing them they may be charged for out-of-network physician services upon admission to a health care facility.

Requires licensed health care facilities to present patients with a document regarding out-of-network physician service charges before rendering services.

Patient may sign the document to indicate review of the information, or a facility employee may sign to verify the patient was presented with the disclosure if the patient declines to sign.

Makes technical corrections to statutory language in section 381.026, changing "any" to "an" and "that" to "which" in various contexts.

Effective date: January 1, 2015.
