Establishes children in out-of-home care have a right to safe, stable, and nurturing relationships with siblings, kith and kin, and parents whose parental rights have been terminated.

Requires caregivers to allow children frequent communication and at least weekly visits with siblings, which cannot be withheld as punishment; court may limit visits only upon clear and convincing evidence of harm.

Mandates the Department of Children and Families provide parents working toward reunification with evidence-based services tailored to their needs and offer children in care longer than 12 months assistance locating and communicating with their parents.

Prohibits placement of children in group homes except as a last resort after all other options are exhausted; requires the department to review group home placements every 30 days for practicability of family placement.

Requires case plans for children in out-of-home placement to include a written description of evidence-based services designed to assist parents in providing safe, stable, and nurturing relationships with their children.
